Breaking Down the Catalyst Concierge Care Assessment

What are we going to do today?

This is probably the most common question we are asked when a new patient comes to Catalyst Physical Therapy. So many people end up in our office referred by friends or family. And in the case where the doctor does recommend physical therapy, they do not necessarily go over how it all works. In this blog post, I am going to break down what you can expect in your first session at Catalyst, and how we use this assessment to create your unique path for care that will ultimately help you create long term, sustainable results.

The assessment, or the first visit, is what we like to think of as an information gathering session. We are learning about you and you are also learning about us, and our approach to physical therapy. It’s in some ways like a first date, or a coffee chat with a new friend. We want to get to know more about you - not only the pain, problem or injury you are dealing with - but why feeling better matters. We want you, through the assessment, to begin to understand the drivers behind the problem, and begin to shed light on a new path forward. We also want you to get to know your therapist, and our unique approach to creating results based care. We want you to leave physical therapy not only feeling better physically but also as a better person.

We begin the assessment by gathering the information we need to understand all the factors that may affect your  problem, and all the barriers to your recovery.  So, we will spend a good amount of that first hour just talking and getting to know each other a little better. Once we know your history and how you ended up in the office, it’s time to do a physical examination. At Catalyst, we take a holistic approach to the body; everything is connected. We do a thorough examination, making sure not to ignore another area of the body that could be contributing to the problem. We check mobility, strength, functional movement and so much more. As physical therapists we are trained to examine all areas of the body and understand how they relate to each other. 

The examination helps us get an idea of what issues may be “driving” the problem and from there, we can decide together how to help you move forward.. The last part of the assessment is spent going over these results and creating a roadmap for your success using our Catalyst Concierge Care model. This model supports your recovery by directly addressing the root problem and creating lasting change without barriers or limitations. This is a fluid process and yes, things do change over time. But this does give us a starting point and we can set some goals to move you from where you are to where you want to be. The plan we develop together will be based in part on what we discover in your assessment. The rest will be up to you and your goals. After all, this IS about your health and well-being.

The last part of the session is your time to ask questions. What is most important to us at Catalyst is that you are crystal clear on the proposed plan, how it works and what each of our roles will be in the process. We want you to know you are an integral part of your own success and that we value your ideas and opinions in addition to our own. 

Together, we will create a program you will feel good about and get you moving toward the life you desire.
